Parental consent and medical consent

I agree to my child participating in the Easter Classic tournament being held at Rosedale Park, Jack Hinton Drive, Rosedale from Friday 19 April - Monday 22 April 2019.

I acknowledge that there are risks associated with involvement in participating in this event and that these risks cannot be completely eliminated. I understand that NHSA will identify any foreseeable risks or hazards and implement correct management procedures to eliminate, isolate or minimise those hazards. I will do my best to ensure that my child follows these procedures.

I understand that NHSA does not accept responsibility for loss or damage to personal property and that it is my responsibility to check my own insurance policy

I agree that if prescribed medication needs to be administered to my child a designated adult will be assigned to do this. I will ensure that prescribed medication is clearly labelled, securely fastened and handed to the designated adult with instructions on its administration.

In the event of my child suffering from a headache or fever, I consent to my child being given paracetamol/panadol

I will inform the manager as soon as possible of any changes in the medical or other circumstances between now and the commencement of the tournament.

I agree to my child receiving any emergency medical, dental or surgical treatment, including anaesthetic or blood transfusion as considered necessary by the medical authorities present.

Any medical costs not covered by ACC or a community service card will be paid by myself/parent/caregiver

Code of Conduct *

Please read and both parent/caregiver and child/young person to sign. By signing your name you agree to the terms in the code of conduct.

1. Keep within the defined boundaries of the playing/coaching or team area

2. Behave and listen to instructions of the representative management team (coach/manager/scorer/leader/parent help if provided)

3. Respect and care for North Harbour Softball Association (NHSA) equipment

4. Not use bad language or racial/sectarian references

5. Not engage in bullying, violence or persistent use of rough and dangerous play

6. Show respect to other children/young people

7. Not use illegal substances, performance enhancing substances and or alcohol

8. Not engage in actions that threaten the safety of myself or others

9. Report inappropriate behaviour or risky situations by youth members

10. Play fairly and not cheat

11. Respect officials and accept officials decisions

12. Show appropriate loyalty to their team and be gracious in defeat

13. Respect opponents

14. Read and agree to follow the Softball NZ Zero tolerance of violence policy

I understand that any misdemeanours or general misbehaviour must be addressed by the immediate coach or manager. The manager will verbally report this to the appropriate representative from NHSA. Should the manager and NHSA representative deem the misconduct serious enough that the representative player needs to be removed from the tournament, their parent/caregiver will be advised and at the cost of their parent/caregiver the representative player will be returned home immediately. The representative player will also be requested to attend a disciplinary meeting with representatives from the NHSA board within 7 days of the tournament fixture finishing.

By signing this document you agree to follow the rules set down by North Harbour Softball Association and Softball New Zealand and understand the consequences of breaching those rules.
